Richland County Restaurant Inspections: Serious Violations

The Richland Public Health Service conducted inspections of the following restaurants for serious violations between October 25 and 27:
● Springmill Stem Elementary School, 1200 Nestor Drive, Mansfield, 25 October. The Responsible Person (CRP) did not ensure that employees properly maintain the temperature of TCS food products during hot and cold storage (critical). The thrown salad and Hobart’s refrigerator had no temperature. TCS food products were not cooled to the correct temperature (critical, correct during inspection). The temperature in Hobart’s refrigerator was 54 degrees when it was opened. One serving of milk has a temperature of 46-47 degrees. Ready-to-eat meals in the freezer are transferred to another freezer. TCS food not chilled to correct temperature (critical, corrected). The temperature for the TSC salad in the serving line is 55 degrees. The item listed has been removed from the service string to be discarded.
● Lexington Court Care Center, 250 Delaware St., Lexington, 25 October. Hand sinks are not only used for washing hands (critical, corrected during inspection). A food service worker was seen filling a glass from the sink. Quaternary ammonium disinfectant solution with incorrect temperature, concentration and/or water hardness (strong repetition). At the time of inspection, the quaternary ammonium disinfectant dosed into a three-compartment sink was approximately 100 ppm. Corrected by November 1st.
● Panera Bread No. 4780, 2070 Walker Lake Road, Mansfield, 25 October. Food products are not adequately protected from contamination by separation, packaging and segregation (critical, correct when tested). The previously discovered fillings cause the thread to open. ● Richland Skyline Inc., 1320 N. Lexington Springmill Road, Mansfield, 25 October. Dirty surfaces or utensils of equipment in contact with food (significant, correct on inspection). Small bowl seen in drawer near service station with visible residue/dry leak in bowl. Disinfectant chlorine solution with wrong temperature and/or concentration (critical, corrected). Please note that the minimum temperature of the dishwasher is 110 degrees. The PIC adjusted the water heater to heat the dishwasher to the desired 120 degrees.
● Lex Lanes, Lex-Luray Enterprises Inc., 60 Ohio 97, Lexington, 26 October. The PIC cannot demonstrate knowledge without committing a serious breach. (critical). Food products are not adequately protected from contamination by separation, packaging and segregation (seriously, correct when checking). Flameball bottles have been seen with the lids closed. Workers measure broccoli without disinfecting the food-contact surfaces of thermometers. TCS food not chilled to correct temperature (critical, corrected). Traulsen two-door refrigerators have 51 degrees of milk. The FAC threw out 30 pints of milk.
● Madison Early Childhood Learning Center, 1035 Grace St., Mansfield, 27 October. Surfaces of equipment or utensils in contact with food are dirty (significant, corrected on inspection). The blade of the can opener was found to have food residue.

Disinfectant chlorine solution with wrong temperature and/or concentration (critical, corrected). A bucket of bleach in a three-compartment sink is 0 ppm. PIC reconstituted the solution to a concentration of 100 ppm. Incorrect storage of poisonous or poisonous materials (seriously, fixed).
